Adding Questions to Your Checkout Form

  1. Access your campaign’s settings: Open the Questions tab and check the Enable questions box.
  2. Add questions: Choose from existing questions or create new ones.
  3. Organize questions: Use the six-dot symbol on the left to rearrange the order, and the three dots on the right to remove a question.

Creating New Questions

To create a new question, enter the desired query and click the Create new question drop-down to open a modal.

Question Code

  • Choose a code for your question: This code labels the question when you export its answer values or search for it in mapping rules for integrated services.
  • Make a question required: Use the Make a response required option to set mandatory questions.
  • Review and manage questions: You can review and create more questions in your main organization settings. Learn more →

Question Type

Choose the type of answer input required:

  • Text line: Single-line text input for simple responses.
  • Text box: Multi-line text input for longer responses.
  • Choose one option: Pre-set responses for selecting one (radio buttons).
  • Dropdown menu: Pre-set answers available in a dropdown menu for single selection.
  • Multiple checkboxes: Allows multiple selections from a list.
  • Single checkboxes: Boolean-style response with one checkbox that can be pre-checked by default.

The Questions Screen

If you enable questions for your campaign, they will be displayed to supporters on the Questions screen after they have donated, either in the Checkout modal or on the Campaign Page. This process ensures there is no loss in conversion since the donation and collection of essential personal information (name, email) are completed beforehand.
